Comprehensive Metabolic Assessment – Patient Summary
Understanding Your Comprehensive Metabolic Assessment
Your body’s metabolism is at the heart of how you feel, move, and function.
This assessment helps you understand how your body is working beneath the surface — not just your weight, but how efficiently you burn fat, build muscle, and use energy.
We use two powerful tools:
ECAL Metabolic Testing: a simple breathing test that measures how your body produces and uses energy in real time.
InBody 570 Body Composition Analysis: a non-invasive scan that shows your unique breakdown of muscle, fat, and hydration, offering insight into overall cellular health.
Together, these tests reveal whether a slow metabolism could be contributing to fatigue or difficulty losing weight, and whether your body is burning fat or glucose as its main energy source — plus how well it can switch between the two.
This allows us to build a data-driven, personalised plan that’s tailored to your physiology. Over time, we can map how your body adapts, showing what’s working, what needs adjusting, and how your exercise and nutrition are truly improving your metabolic health.
How We Use Your Results
Your Comprehensive Metabolic Assessment is more than a single test — it’s your personal roadmap to better health.
Each re-test allows us to track how your body responds to movement, nutrition, medications, and lifestyle changes.
We use this data to fine-tune your program so that every decision — from training intensity to recovery strategy — is based on evidence, not guesswork.
This means you can:
- See what’s happening inside your body in real time.
- Know exactly what’s working for you.
- Stay motivated by watching measurable progress unfold.
What to Expect During Your Assessment
Your appointment takes around 45 minutes and is completely non-invasive. It includes both the ECAL metabolic breath test and the InBody 570 body-composition scan.
Before your appointment:
- Fast for at least 4 hours (water is fine).
- Avoid exercise, caffeine, and anything that raises body temperature for 12 hours before testing.
- Wear light, comfortable clothing and remove any metal jewellery.
During the assessment: - You’ll complete a short health and lifestyle questionnaire.
- The InBody 570 scan uses a harmless, painless electrical current to measure muscle, fat, and hydration.
- If you have a pacemaker or implantable device, this step will be skipped for safety — but you can still complete the ECAL metabolic breath test, which provides a full metabolic assessment.
- he ECAL test involves breathing normally through a mouthpiece for 3–5 minutes while relaxed. It measures oxygen and carbon-dioxide levels to determine how efficiently your body produces and uses energy.
After the assessment:
Your Exercise Physiologist will explain your results clearly and help you understand what your numbers mean. You’ll receive an individualised plan with exercise, nutrition, and recovery recommendations designed to improve fat-burning efficiency, energy, and overall health.
How It Can Help You
Fertility & Pre-Conception
We prepare your body for pregnancy by improving energy efficiency, muscle strength, and hormone balance. This helps support healthy ovulation and lowers the risk of gestational diabetes.
For those undergoing fertility treatment (like IVF or ovulation induction), we can monitor energy stability and tailor exercise to complement medical care.
Polycystic Ovary Syndrome (PCOS)
We design targeted exercise and lifestyle plans to improve insulin sensitivity and fat metabolism. Over time, you’ll notice better energy, fewer symptoms, and improved body composition.
Perimenopause & Menopause
We track metabolic rate, muscle mass, and bone density to offset the natural slowdown that occurs during this stage. Your program focuses on maintaining strength, energy, confidence, and long-term health.
GLP-1 Therapy Users
We help monitor changes in metabolism and muscle mass to ensure your weight loss is safe, effective, and sustainable. If needed, your plan is adjusted to preserve muscle, energy, and metabolic function.
General Weight Management & Lifestyle Optimisation
If you’ve struggled with stubborn weight or low energy, this assessment shows whether your metabolism is working efficiently.
It helps determine if you’re primarily burning fat or glucose, how well you switch between fuels, and whether your nutrition and movement plan are helping your body burn fat better.

Comprehensive Metabolic Assessment – Clinical Summary
(For GPs, Endocrinologists, and Women’s Health Specialists)
Overview
The Comprehensive Metabolic Assessment combines ECAL metabolic testing and InBody 570 body-composition analysis to deliver a complete, evidence-based picture of metabolic function, cellular health, and body composition.
This dual-system approach allows precise, data-driven exercise, nutrition, and medical interventions for fertility, PCOS, menopause, weight management, and GLP-1 therapy.
For women’s health practitioners, metabolic testing provides a critical diagnostic and monitoring tool that complements hormonal and biochemical assessments. Female metabolism is highly dynamic—changing with menstrual cycles, pregnancy, contraceptive use, and menopause—and these shifts directly affect insulin sensitivity, substrate use, and resting metabolic rate.
Key reasons this testing is essential include:
- Personalised hormone and fertility care: Identifies impaired fat oxidation and insulin resistance that can disrupt ovulation and hormonal balance, enabling targeted pre-conception and fertility treatment plans.
- PCOS management: Objectively measures the impact of exercise, dietary changes and GLP-1 or metformin therapy on metabolic flexibility, allowing clinicians to quantify improvements in insulin control and body composition.
- Perimenopausal and menopausal transition: Tracks the metabolic slowdown (~10-25%) and changes in body-fat distribution, helping clinicians mitigate weight gain, protect muscle and bone mass, and manage symptoms through precise lifestyle prescriptions.
- Medication monitoring: Provides data to inform dose optimisation and safety decisions when using GLP-1 medications or hormone replacement therapy—ensuring lean-mass preservation and adequate energy availability.
- Long-term health protection: Detects early shifts in metabolic efficiency that predict future risk for type 2 diabetes, cardiovascular disease, and osteoporosis, supporting preventive, whole-body care.
Why Baseline Testing Matters
- Establishes objective metabolic and body-composition starting points.
- Detects suppressed metabolism or lean-mass loss early.
- Tracks adaptations from exercise, nutrition, or GLP-1 medication.
- Provides measurable outcomes for collaborative clinical decision-making.
- Enhances patient motivation and engagement by allowing patients to see tangible changes in their data over time.
- Understanding their numbers helps patients take ownership of their progress, stay accountable, and gain confidence that their health and metabolism are genuinely improving.
- Empowers both the Exercise Physiologist and the broader healthcare team to make timely, informed adjustments to exercise, medication, or nutrition plans. This data-driven approach ensures each intervention remains relevant and effective — because ultimately, you can’t manage what you can’t measure.
ECAL – Metabolic Efficiency and Energy Use
- Resting Energy Expenditure (REE): True metabolic rate (kcal/day).
- Substrate Utilisation & Respiratory Exchange Ratio (RER): Fat vs carbohydrate use and metabolic flexibility.
- O₂ Consumption / CO₂ Output (VO₂, VCO₂): Aerobic capacity and mitochondrial function.
- Fat Oxidation Rate: Ability to burn stored fat efficiently.
InBody 570 – Body Composition and Cellular Health
Interpreted clinically by Exercise Physiologists to track muscle, fat, fluid, and inflammation.
- Body Composition Analysis: Total body water, protein, minerals, and body-fat mass.
- Differentiates true weight change from fluid retention or oedema (cardiac / inflammatory).
- ECW ratio ↑ = systemic inflammation or fluid overload.
- Muscle–Fat Analysis: Visual C–I–D Shape model.
- C-shape = low muscle / high fat; I-shape = balanced; D-shape = optimal (high muscle, low fat).
- Simplifies patient feedback and goal-setting without numeric fixation.
- Segmental Analysis: Compares muscle and fat distribution per limb for imbalance, post-surgery, or injury tracking.
- Highlights limb discrepancies to target rehabilitation or strength work.
- Visceral Fat Level: Key marker for cardiometabolic risk and long-term health.
- Bone Mineral Content & Phase Angle: Trends bone density and cellular integrity for aging or osteopenic patients.
Clinical Applications
Fertility & Pre-Conception
Optimises insulin sensitivity, fat oxidation, and muscle integrity to support hormone balance and reduce gestational-diabetes risk.
Also assists with hormone management during fertility treatment programs (e.g., IVF or ovulation induction), helping to stabilise energy metabolism, improve treatment responsiveness, and enhance overall reproductive-health outcomes.
Polycystic Ovary Syndrome (PCOS)
Combines metabolic and composition data to guide insulin-resistance management and body-recomposition strategies through targeted exercise.
Perimenopause & Menopause
Tracks metabolic-rate and muscle-mass decline (~25%) to preserve lean tissue, bone density, and energy expenditure through individualised strength programs.
GLP-1 Therapy Users
Detects early reductions in metabolic rate and muscle mass during rapid weight loss. Facilitates dose review and exercise intervention to maintain metabolic health.
General Weight-Loss and Lifestyle Clients
Quantifies fat-loss vs muscle-loss to ensure safe, sustainable progress. Provides visual trending (C → I → D shape) and objective feedback to improve motivation and accountability.
Role of Exercise Physiology
Accredited Exercise Physiologists interpret ECAL + InBody data to:
- Prescribe evidence-based movement, exercise and lifestyle programs that target fat oxidation, preserve lean muscle mass, and enhance metabolic efficiency.
- Guide individual energy targets and expenditure recommendations based on each client’s measured metabolic rate and fat-burning capacity, optimising their ability to utilise fat as a primary energy source. This, in turn, improves insulin sensitivity and supports hormonal health, particularly in clients managing conditions such as PCOS, menopause, and metabolic dysfunction.
- Design holistic behaviour and lifestyle programs incorporating structured exercise, recovery strategies, sleep optimisation, and stress regulation.
- Monitor segmental imbalances and guide rehabilitation or strength progression following injury, surgery or to make exercise programs accessible to every body.
- Re-test every 2–12 weeks, tailoring intervals to the client’s medication use, clinical presentation, and goals. This ensures a reactive approach—allowing early intervention when medication dosage or physiological response changes—to optimise safety, progression, and outcomes.
- Communicate results clearly with the referring GP or specialist to support coordinated multidisciplinary care.
Integrated Care Model
O2 Active Exercise Physiology uses data-driven assessment to link exercise prescription, energy guidance, nutrition education, and medical management into one comprehensive, evidence-based program.
The Comprehensive Metabolic Assessment ensures patients achieve sustainable improvements in metabolic efficiency, body composition, and overall health span.

Understanding Your Metabolism: The Missing Piece in Health, Hormones, and Longevity
Many of us have been told to “eat less and move more,” but if that worked for everyone, none of us would struggle with weight, energy, or hormonal changes. The truth is, your metabolism — how your body produces and uses energy — is at the centre of everything from fertility and menopause to how easily you lose fat or feel energised throughout the day.
At O2 Active, our Comprehensive Metabolic Assessment combines the ECAL metabolic test with an InBody 570 body-composition scan to uncover why your body feels stuck, tired, or resistant to change — and what to do about it.
Why Testing Matters Before You Start a Program
When people start weight-loss medication, IVF, or even a new fitness plan, they often forget one key step: establishing a baseline.
Knowing your metabolic rate and fat-burning capacity before you start means we can track exactly how your body is adapting — not guessing.
Whether you’re starting GLP-1 therapy, preparing for fertility treatment, or entering menopause, these numbers give your healthcare team data to guide decisions safely and effectively. An orthopaedic surgeon once said it best:
“My surgical outcomes improve if my patients lose 10 kg — but they do even better when I help them improve their metabolism first.”
Your metabolism influences how well you respond to every intervention — from medication to exercise.
How It Works
The ECAL test is a 3–5-minute breathing test that measures how efficiently your body burns fat or carbohydrates at rest.
The InBody 570 scan shows what’s behind the number on the scales — muscle, fat, water, bone, and inflammation.
Together, they help us answer questions like:
- Is my metabolism running slow?
- Is my body burning fat or sugar as its main fuel?
- Am I losing muscle while trying to lose weight?
- How is my body adapting to medication or lifestyle changes?
For Women at Every Stage Fertility & Pre-Conception:
We help optimise insulin sensitivity and fat oxidation to support hormonal balance and egg health — crucial for those undergoing IVF or fertility treatments.
PCOS:
By understanding your metabolic profile, we can tailor exercise and lifestyle strategies to improve insulin control and reduce inflammation.
Perimenopause & Menopause:
During this transition, metabolism slows by up to 25%. Testing helps you preserve muscle, bone density, and energy, while preventing the weight gain and fatigue so many women experience.
- For Those Using GLP-1 Medications
If you’re using medications such as Ozempic, Wegovy, or Mounjaro, regular testing is vital.
Rapid weight loss can reduce muscle and slow your metabolism — sometimes by more than half. Monitoring helps your medical team stay proactive, not reactive, adjusting dosages and exercise to protect your long-term health. - For Active Adults and Everyday Health Seekers
Even for regular exercisers or athletes, metabolic testing highlights how efficiently your body uses energy. For example, many fit people are still carb-dependent — relying on sugar instead of fat as fuel.
By improving fat adaptation, you’ll enjoy steadier energy, better endurance, and improved recovery. - Adapting Over Time
Your metabolism changes with lifestyle, medication, stress, and age.
That’s why we offer optional re-testing every 2–12 weeks — to see how your body is adapting and fine-tune your plan.
This empowers you to take charge of your health while giving your clinician the data to adjust your care confidently. - Building a Longevity Mindset

What is metabolic health?
Your metabolism is your body’s way of turning food into energy. We need this energy to do everything from breathing to thinking to moving.
Metabolism involves a series of fine-tuned chemical processes. When these processes are running smoothly, they supply our cells with all the energy they need to work and stay healthy.
When you define metabolic health, it’s about more than reaching a healthy weight – metabolic health also means you have healthy levels of:
- body fat and lean muscle mass
- blood glucose (blood sugar)
- blood fats (triglycerides)
- cholesterol
- blood pressure
Wondering how to improve your metabolic health?
A healthy metabolism involves a balancing act between building up your tissues and energy stores and breaking them down to fuel body functions.
Getting this balance right can help you to have good metabolic health.
Good metabolic health can lower your risk of many chronic health conditions and help you stay healthy throughout life.
At our Perth metabolic health clinic, we use medical grade devices to measure your metabolism. We use this data – and a scientifically proven approach to boosting metabolic function – to design a dietary and exercise program specifically for you.
Improving metabolic health: How the program works
Our highly trained exercise physiologists will work closely with you to understand your metabolism and optimise it to burn fat, produce energy, and improve your health.
Here’s a step-by-step guide to how our metabolic health program works.
Step 1. Metabolic health assessment
First, we take a look at your markers of metabolic health, including the usual things like your weight, muscle mass, body composition, waist circumference and previous blood test results.
But the key to unlocking your body’s fat burning ability involves understanding your metabolism at a cellular level.
To do this, we use world-first, clinically proven technology called ECAL – a portable metabolic monitor that gathers information about your:
-
- daily energy needs at rest
- how this energy is being produced (from fat or carbohydrate)
- how efficiently your cells use oxygen to produce energy.
ECAL assessment is easy, non-invasive and painless – you simply breathe into the device for about 8 minutes and it analyses your metabolic health markers.
ECAL technology was previously only available to elite athletes and academics. Now we can use it to get information about your personal metabolic profile and how to optimise your metabolic function.
Step 2. Program design
Programs last from 8 to 16 weeks. Throughout, you’ll receive support from your exercise physiologist, including:
-
- help with setting realistic, meaningful and achievable goals
- accountability, tips and encouragement to help you stay on track
- advice to help you make small, sustainable lifestyle changes that support better health.
Step 3. Review and results
Throughout your program, we’ll retest your metabolic health. Not only will you learn how your weight and body composition have changed, you’ll get to see how your body is learning to burn fat better through dietary and lifestyle changes.
Our clinical programs are designed to complement medical management. We work closely with your doctor and other health professionals to help you get the best possible results.
