Our Chronic Disease Management Program allows you to make small sustainable changes to your daily routine and activity levels can create meaningful change in reducing the impact of your chronic disease or condition.

Chronic disease is a broad range of complex and long-term health issues, and are linked to multiple causes and result in a gradual deterioration of quality of life and overall health. 50% of all Australians suffer from at least one of the 8 common chronic diseases including arthritis, asthma, back pain, cancer, cardiovascular disease, chronic obstructive pulmonary disease (COPD), diabetes, and mental health conditions, with 23% of Australians having a diagnosis of two or more conditions.

Physical inactivity is the 4th largest cause of chronic conditions in Australia. If people were sufficiently active, it could cut the rate of many chronic diseases by as much as 40%.

Our exercise physiologosts work collaboratively with other health professionals and prescribed exercise  and education to help you manage your symptoms, slow the progression and improve your qulaity of life.  This enables you to improve your health, have some control over your disease and decrease the burden your chronic disease or condition has on your life. We assess, goal set, prescribe, educate, supervise and monitor exercise and lifestyle programs and interventions.

 As much as 53% of diabetes burden and 45% of osteoarthritis burden are due to overweight and obesity alone.

Exercise can also help to reduce other risk factors for chronic diseases, such as:
• High blood pressure
• High cholesterol
• High body mass

Our clinical porgrams are complimentary to medical management, and aim to improve patient outcomes and reduce medication needs.

 

  • 2. EXERCISE PRESCRIPTION

  • Clinical, home-based, gym-based, group and/or hydrotherapy.

  • Application of strength, balance, core, functional and flexibility exercises.

    3. REVIEW AND PROGRESSION

  • Client feedback and AEP assessment.

  • Program enhancement and modification.

  • Review, advise, lifestyle modification and ongoing support.

    4. LONG TERM MANAGEMENT

  • For patients with ongoing chronic issues or general strength and exercise requirements post rehabilitation; group exercise classes
    as part of a long term maintenance program are available. Health rebates may apply. O2 Active aims to help you incorporate exercise into your everyday lifestyle in a supportive environment that benefits participants health and social wellness.

  • Group classes provide a sustainable and affordable progression path for long term physical maintenance, education and support.

REFERRAL PATHWAYS

Eligible patients can be referred to AEP’s by general practitioners under a GP management plan or team care arrangement. A chronic disease management plan can be available for individual or group allied health services, including type 2 diabetes group exercise sessions.

Patients may be eligible for Medicare (GP TCA or diabetes group referral required), NDIS, Work Cover, Department of Veterans Affairs, TCA and Health Insurance rebates for exercise physiology services.

HOW WE REPORT BACK TO REFERRING PRACTITIONERS

• Written initial and final reports including patient’s assessment and results.

• Prescribed intervention and progressions, where appropriate.

• Keep all referring and treating practitioners up to date with their patient’s outcomes.

“20 years ago, when I was in my late forties, I began having trouble with my back, and was advised to start exercising on a regular basis. I started going to a Pilates class, which was run run by Caitlin, who had graduated from university with a degree in sport science.

Fast forward 18 years of 3-4 classes a week (yoga and pilates) with Caitlin, when I was diagnosed with severe spinal stenosis, and needed spinal surgery. I was told by the surgeon that I needed a double laminectomy, but, because my back muscles were so strong due to my exercise regime I would not need a spinal fusion.

    Improving Strength and Movement Tolerance: Improving functional strength and movement capabilities.
    Improved Mobility: Exercise helps encourage various planes of movement increasing range and functionality of the lower back.
    Reduced Pain and Discomfort: Regular movement helps with management of chronic pain.

    Cancer

    Improved Physical Function and Strength: Exercise helps maintain muscle mass and improve overall strength.
    Reduced Fatigue: Physical activity can assist with management of cancer-related fatigue.
    Enhanced Mental Health: Exercise reduces anxiety, depression, and stress, improving overall quality of life.

    Cardiovascular Disease

    • Improved Heart Health: Exercise strengthens the heart, improving its efficiency in the heart’s ability to pump blood to your lungs and throughout your body.
    • Lower Blood Pressure and Cholesterol: Regular physical activity helps manage blood pressure and cholesterol levels.
    • Enhanced Circulation: Increased blood flow benefits the entire cardiovascular system.

    Chronic Obstructive Pulmonary Disease (COPD)

    How to become involved in the CHRONIC DISEASE MANAGEMENT PROGRAM: 

    STEP 1: Call O2 Active assessment

    Call O2 Active to learn more about our Chronic Disease Management program. Our staff will be ready to answer your questions and book your initial 45 – 60 minute consultation with an Exercise Physiologist. Medicare and health rebates may apply.   

    STEP 2: Initial consultation and assessment

    First consultation – During this 45-60-minute appointment, we will provide you with some education on how exercise will help your condition or disease. We will discuss your support strategies and your health history including any  omorbidities, musculoskeletal, and mental health; this will allow us to provide you with a holistic approach to your exercise prescription.  

    • We will discuss your goals and build these into your program. 
    • Together, we will conduct an initial assessment, which will guide your optimal starting level.

    STEP 3: First session with child

    Book your first 1 on 1 30 minute Youth Exercise Physiology session where introduce you to your program. 

    We will conduct an initial session taking them through their program. This one on one session will guide us on how best to work with your child and assist in the path we take to ensure successful outcomes in the group setting.  

     

     

    STEP 4: Weekly sessions

    Book into the group Neurodiverse Program of weekly session/s. The group will have a ratio of 4:1 of children to therapist to ensure the best care and outcomes are achieved.
