If you’re in the Eastern Wheatbelt, we run sessions through the Integrated Chronic Disease Care (ICDC) program at the Northam GP Super Clinic.

What is chronic disease management?

A chronic disease is a condition that does not go away. Chronic diseases often lead to a gradual decline in health, wellbeing and quality of life.

It’s estimated that 60% of Australians live with at least one long-term health condition. Many people have more than one. The percentage of people with a chronic disease rises with age, up to 94% of those aged 85 and older.

Chronic disease management services Perth – Your pathway with 02 Active

Here’s what to expect if you take part in a chronic disease management program with us.

Step 1: Your first visit

This visit is about getting to know you. We’ll take the time in a one-on-one consult to learn about your condition, your current level of function, and the things that are most important to you. We’ll help you set goals that are realistic, achievable and meaningful, and design a program to help you reach them.

You can choose to keep seeing us at our clinic one on one, join a clinical group class, use our open gym, or do a program on your own at home. If you have opted for a longer appointment, we’ll give you some exercises to take home and get started.

Step 2: Exercise therapy

Next, we’ll support and monitor you as you work on your program. Depending on your goals, this will include a mix of strength, balance, core, functional and flexibility exercises. You can visit us at our state-of-the-art North Fremantle gym, or we can see you at home. We also offer hydrotherapy sessions. 

Step 3: Review and progression

As you get fitter and stronger, it’s important that we check in with you and update your program. We’ll keep challenging you towards your full potential, with continued monitoring to ensure you are doing your exercises safely and effectively. We’ll also provide advice and support to make healthy lifestyle changes and adjust your program as your health changes.

Step 4: Long term management

If you have a chronic condition, being active is a key way to maintain your health, strength, functional ability and quality of life.

Our group exercise classes are an affordable way to stay active in a safe, supportive and motivating environment. Group classes are also great for your social wellness.

How to start your chronic disease management plan with O2 Active

GPs can refer eligible patients to our AEPs under a GP management plan or team care arrangement. A Medicare chronic disease management plan can be used for individual or group allied health services, including type 2 diabetes group exercise sessions.

You may be eligible for rebates for exercise physiology services through Medicare, Work Cover, Department of Veterans Affairs, TCA, private health Insurance cover or the NDIS.
